Who writes this

I am Blane Jackson. Doctor of Dental Surgery from A.T. Still University's Arizona School of Dentistry & Oral Health. MBA with a healthcare concentration. I am a practicing dentist and multi-practice owner/operator with firsthand experience reviewing DSO term sheets, APAs, earnouts, rollover equity, and post-close operating terms.

I sat across from term sheets from several of the DSOs profiled on this site. I signed one of them. I also did not sign several others, for reasons I wish I had known earlier in the process. This site is an attempt to write those reasons down.

Methodology

Each DSO profile is scored out of 100 across five factors: offer competitiveness, post-close clinical autonomy, contract fairness, earnout mechanics, and post-close culture. Weights are roughly equal with a slight emphasis on contract fairness and earnout mechanics, because those are the areas where most sellers leave real money on the table.

Scoring inputs come from three sources: (1) publicly disclosed transaction documents and SEC filings where available, (2) direct communications with sellers who completed transactions with each DSO in the 2024–2026 window, and (3) my own review of term sheets and APAs during my own practice sale process.

I do not take payments, placements, broker commissions, or buyer-side referral fees from any DSO profiled here. I do not provide brokerage services. If a third-party relationship ever appears on a page, it is disclosed on that page.
